Am I a Good Candidate?
A BBL may be appropriate for those who:
- Are in good overall physical health and at a stable weight
- Are a non-smoker, or willing to cease smoking at least 4–6 weeks prior to surgery
- Have sufficient donor fat available in areas such as the abdomen, flanks, or thighs
- Are dissatisfied with the size, shape, or projection of the buttocks
- Are willing to follow strict post-operative positioning instructions
- Have realistic expectations about the degree of augmentation achievable with fat grafting
What to Expect — The Procedure
The procedure involves liposuction at donor sites, purification of the harvested fat, and injection into the buttocks in multiple layers to achieve shape and volume. Current safety guidelines recommend that fat injections be placed into the subcutaneous layer only, which significantly reduces the risk of serious complications. Confirm that your surgeon follows current BBL safety guidelines.
Pre-operative Care
- Cease smoking at least 4–6 weeks prior to surgery
- Avoid blood-thinning medications and supplements (aspirin, ibuprofen, fish oil, vitamin E) for 2 weeks before surgery
- Inform your surgeon of all current medications, allergies, and medical conditions
- Maintain a stable weight prior to surgery — significant weight loss after a BBL can reduce surviving fat volume
- Arrange a BBL pillow or donut cushion in advance — essential for avoiding pressure on the buttocks post-surgery
- Prepare loose-fitting clothing, particularly bottoms with easy access

Post-operative Care
- Wear any prescribed compression garment as directed
- Avoid sitting or lying directly on the buttocks for 6–8 weeks — use a BBL pillow to transfer pressure to the thighs
- Sleep on your stomach or side during the recovery period
- Wear the compression garment on the liposuction donor sites as directed
- Avoid vigorous exercise, particularly lower body workouts, for at least 6–8 weeks
- Maintain a stable body weight during recovery — significant weight loss can reduce the volume of surviving fat
When to Call Your Surgeon
- High fever (above 38.3°C / 101°F)
- Sudden or rapidly increasing pain not relieved by prescribed medication
- Excessive bleeding or unusual discharge from the incision site
- Sudden swelling in one leg noticeably different from the other — may indicate a blood clot
- Sudden significant asymmetry, hardening, or severe pain in the buttock area
- Signs of allergic reaction — skin rash, severe itching, swelling, or difficulty breathing
